Assumption of Participation Risks:
By participating in activities with Healing Connections, I understand that, although our experiences are designed with care and attention to safety, there may still be minor risks involved. These might include occasional physical discomfort, unexpected strains during movement, or moments of emotional vulnerability. I voluntarily accept these potential risks as part of my involvement in any events, workshops, or programs organized by Healing Connections throughout the calendar year in which this form is signed.
Release, Waiver, and Indemnification:
In consideration of being allowed to participate in Healing Connections activities, I, along with my heirs and assigns, hereby waive, release, and discharge Healing Connections, its directors, staff, and representatives from any liability for any injury, loss, or damage (including any unforeseen emotional distress) that might occur during my participation. I agree to indemnify and hold harmless Healing Connections from any claims, costs, or expenses (including reasonable legal fees) arising from my involvement, to the fullest extent permitted by law.
Acknowledgement of Understanding:
I acknowledge that I have read and fully understand the terms of this waiver, and I am aware that by signing below I am relinquishing certain legal rights, including the right to sue. I confirm that my participation is completely voluntary, that I am 18 years of age or older, and that I am of sound mind.
Confidentiality of Event Details:
I understand that specific details about the locations and logistics of Healing Connections events are provided solely for my participation and the safety of all involved. I agree to keep this information confidential and not to share it with any third parties.
